Volunteer & Contractor Waiver, Release, and Agreement
This agreement releases Divine Empowerment Church, a non-profit corporation organized and existing under the laws of the State of Florida, and each of its directors, officers, employees, agents, and representatives from liability. The volunteer or contractor desires to provide services for Divine Empowerment Church and engage in activities related to serving as a volunteer or contractor by performing associated duties.
A Volunteer is defined as: foster parents, community service workers (court-ordered or not), church members, or a vendor who is participating in on-site functions without compensation. A Contractor is any individual or vendor, paid or unpaid, who has agreed to provide specific services or duties for a designated period.
Volunteer Understanding:
Volunteers understand that their relationship with Divine Empowerment Church is limited to a volunteer role and that no compensation is expected in return for services. Divine Empowerment Church will not provide any benefits traditionally associated with employment, and the volunteer is solely responsible for their own insurance coverage in the event of personal injury, illness, or damage incurred while serving.
Contractor Understanding:
Contractors (whether paid or donating services) understand that their relationship with Divine Empowerment Church is limited to the agreed-upon scope, duties, and hours. Compensation will be provided as agreed. Divine Empowerment Church does not provide benefits traditionally associated with employment, and the contractor is responsible for their own insurance coverage in the event of personal injury, illness, or property damage while performing services.
1. Waiver and Release:
I, the undersigned volunteer or contractor, release and forever discharge and hold harmless Divine Empowerment Church, its directors, officers, staff, and assigns from any and all liability, claims, and demands of any kind or nature, whether in law or equity, arising out of the services I provide or while I am on church premises. This includes any bodily injury, illness, death, or property damage that may result from my services.
2. Insurance:
I understand that Divine Empowerment Church does not provide medical, health, or disability insurance coverage of any kind. I am personally responsible for my own coverage in the event of injury or illness while volunteering or contracting.
3. Medical Treatment:
I hereby release and discharge Divine Empowerment Church from any claim arising from first-aid treatment or medical services rendered in connection with an emergency during my time of service.
4. Assumption of Risk:
I understand that some tasks (including but not limited to lifting, setup, or working with equipment) may involve potential hazards. I expressly assume any and all risks associated with such activities and release Divine Empowerment Church from liability for injury, illness, or property damage arising from them.
5. Media/Photographic Release:
I grant Divine Empowerment Church full rights to use any photographs, video, or audio recordings of me captured during the course of my volunteer/contractor service for church-related promotional, media, or archival purposes, without compensation.
6. Dress Code & Conduct:
As a volunteer or contractor, I understand that I represent Divine Empowerment Church. I will maintain clean, professional grooming and attire. Foul language, disruptive behavior, or unprofessional conduct is not permitted. I will treat all church members, guests, and team members with kindness and respect.
7. Legal Scope:
This agreement shall be governed by and interpreted in accordance with the laws of the State of Florida. If any provision is found to be invalid or unenforceable, the remainder of the agreement shall continue in full force and effect.
